Required deliverables to be provided by applicant:

  • 17-digit Vehicle Identification Number (VIN)
  • Photo of the visible VIN under windshield that clearly shows the entire VIN of the vehicle
  • Photo of the Manufacturer’s Label that clearly shows all the information
  • Photo of the (damaged) Vehicle Emission Control Information label, or if missing, a photo that shows that the label is missing
  • Reason of ordering a new Vehicle Emission Control Information label
  • Copy of existing vehicle registration documents
  • Copy of vehicle owner’s ID card (e.g. copy of passport or driver’s license)
  • Company’s VAT number (If ordered by a company with a registered VAT number in the EU)
  • Shipping and billing address
  • Phone number of shipping address
